When filling out the NJ Driver License Application Request form, applicants often overlook crucial details that can delay or invalidate their requests. One common mistake is submitting photocopies of the form without the original signature of the applicant. The instructions state that while photocopies of the form itself can be used for multiple requests, each submission must include the original signature of the person making the request. This oversight can result in the rejection of the application, as only forms with original signatures are considered valid.
Another frequent error is related to the payment method. The form specifies that payments must be made via check or money order payable to the "New Jersey Motor Vehicle Commission." However, applicants sometimes attempt to make payments using cash, or by credit or debit cards, which the form explicitly prohibits.
